Attaching a failing test for mixed file uploads. I don't have a solution yet.

The one thing that bugs me a little is that we gobble the exception when the stream can't be rewound. Ideally, we'd have some mechanism for at least issuing a warning.

It would be nice if we could log out to rack.errors but Apache/CGI uploads would always produce this error.
